VA’s Community Care Network (CCN) is the link between VA and over a million community providers nationwide who provide timely, high-quality care to Veterans. CCN is made up of five regional networks of community providers that are managed by third-party administrators (TPAs) on behalf of VA. Optum serves as TPA for regions 1-3, while TriWest serves as the TPA for regions 4 and 5.

Region 1: Connecticut, Delaware, District of Columbia, Maine, Maryland, Massachusetts, New Hampshire, New Jersey, New York, North Carolina, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia

Region 2: Illinois, Indiana, Iowa, Kansas, Kentucky, Michigan, Minnesota, Missouri, Nebraska, North Dakota, Ohio, South Dakota, Wisconsin

Region 3: Alabama, Arkansas, Florida, Georgia, Louisiana, Mississippi, Oklahoma, Puerto Rico, South Carolina, Tennessee, Virgin Islands (U.S.)

Region 4: American Samoa, Arizona, California, Colorado, Guam, Hawaii, Idaho, Montana, New Mexico, Nevada, Northern Mariana, Islands, Oregon, Texas, Utah, Washington, Wyoming

High Performing Providers

VA designates certain CCN community providers as High Performing Providers (HPPs). HPPs are community providers who consistently meet or exceed a standard set of performance measures that are set by TPAs.

Performance measures include clinical outcomes, patient satisfaction, cost-effectiveness, and adherence to established guidelines. HPP designation only applies to a certain subset of specialties, which varies by TPA. Community providers do not elect or apply for the designation, and not all providers are eligible for HPP.

VA staff consider the HPP designation during the Veteran appointment scheduling process to help Veterans select community providers that best meet their needs.
