What is a Privacy Complaint?

A privacy complaint is when an individual submits a verbal or written statement to a Privacy Officer, regarding an alleged privacy violation. If you believe you have witnessed a privacy violation of VA Sensitive Information, you should report it as soon as possible.

Follow These Steps to Report a Privacy Complaint

Step 1. Locate and Contact Your Local Privacy Officer

Report the privacy complaint to your local Privacy Officer immediately in person, or by secure email or phone.

Please use the Find VA Locations Tool to find your local VA Facility by entering your location (street, city, state, or postal code) and facility type, then click search to find the relevant facilities.

  • If you are looking for a VA health facility, select VA health.
  • If you are looking for a VA regional benefits office, select VA benefits.

After you have located your VA health or regional benefits facility, call the listed main number and request to speak with your local Privacy Officer.

For the National Cemetery Administration, contact their Privacy Office at NCAPRAD@va.gov.

Step 2. Provide as much detail as possible

  • Your name
  • Contact information
  • Location
  • Date of suspected privacy violation
  • Who was involved in the privacy violation?
  • What happened? E.g. you received another individual’s information or witnessed another individual accessing information
